I was creating a new form with Form Builder, here is the form and I didn't touch it after the bug:

Generate CRUD
-Use Generated List

Generate Approval Process
-Nothing selected (never let me check on this after the 1st use)

Then, in App Generator I did selected the following:
**Generate List
-Default options
**Generate CRUD
-Use Generated List
-Userview: Gestión de la Producción - TEXMEC S.A.C.
**Generate Approval Process
-Nothing selected (can't check on after the first use)
After this, I wanted to launch the user view and I got the following error:
Page Not Found (Code 404)

All the already test I was doing got lost, and, this test was like my real implementation to take it to production. Please hope you can restore that Userview.
