Generate hyperlink using corresponding row data (data-list)

Need to provide download in datalist view. (Download file updloaded via form)
How to generate dynamic hyperlink with the data from corresponding row.

      I googled and found this Download PDF Datalist Action Plugin which might be useful.

        Plugin only put form content in pdf. But am looking to download actual files which are uploaded (File upload). Files are present in relative path as shown in image.
