Judging from the naming you used in the "Form Upload Fields" property in Email Tool, I'm guessing these field IDs are not "File Upload" type form elements.
From the Email Tool - Configure Email Attachments KB page, the "Form Upload Fields" property is used when you need to attach the actual file in a File Upload type form field that exist in your specified form.
It seems that for your use case, this process tool Export Form Email Tool may be more suitable for your requirement, since this tool will attach the form along with its form data as a PDF attachment in an email.
