How to send an email with attachments from list

Hi, 

Recently I have been able to use plugin email tool to send an email, but unfortunately I was unable to send that same email with an attachment with list. 

Can anybody help me how to solve this?

    here is how i have had configure the form, do i need to add form upload fields and files as well?

    I see that that you have not configured both Form Upload Fields and Files.
    Please do that and it should work. if you want to see that is done please follow the link bellow. Do check the demo apps provided
